Getting Started

Follow these steps to begin your child’s journey with Next Level Pediatric Therapy.
 

1. Physician Referral

An updated physician referral is the first step in getting started. Download our referral form and send it to your child’s physician to complete. Your physician can email or fax the completed form directly to our clinic.

2. Complete Your Intake Paperwork

Please download and complete the following PDF forms before your child’s evaluation whenever possible.

• Consent & Policies: Review and complete our required consent forms and office policies.

• Patient Intake Forms: Share information about your child’s medical history, developmental background, current concerns, and therapy goals.

3. Email Your Forms

Once completed, please email your intake paperwork to info@nextlevelpeds.com.

Your child’s physician may email or fax the completed referral form directly to our clinic.

Please contact us if you have any questions about completing or submitting your forms.
 

4. Schedule Your Child’s Evaluation

We are scheduling evaluations for our opening date of November 9, 2026, or shortly thereafter.

Our Clinic Hours

• Monday–Thursday: 8:00 AM–6:00 PM

• Friday: 8:00 AM–4:00 PM
   

Email info@nextlevelpeds.com to discuss scheduling. Please let us know your preferred days and times, whether you prefer morning or afternoon appointments, and any scheduling restrictions we should consider.

We will do our best to accommodate your family’s needs!
 

5. Your First Visit

We can’t wait to meet your family!
 

During your child’s initial evaluation, our therapist will discuss your concerns and goals, assess relevant skills, and determine the most appropriate plan of care.
 

Please arrive a few minutes early and bring any relevant medical information, equipment your child uses, and questions you would like to discuss with your therapist.
 

Our goal is to create a supportive environment where your child feels encouraged to explore their abilities, build confidence, and make meaningful progress.
 

Insurance & Payment

We want to help families understand their coverage and potential costs before beginning services.
 

Insurance

We currently can accept Medicaid and are finalizing credentialing with Blue Cross Blue Shield. During this process, we will make every effort to submit insurance claims when applicable.
 

Coverage and reimbursement depend on your individual plan and our credentialing status. Please contact our office to discuss your insurance coverage and potential financial responsibility before beginning services.
 

We will communicate updates as our credentialing is finalized.
 

Self-Pay Rates

For families paying out of pocket, our standard rates are:

• Initial evaluation: $175

• Individual treatment session: $125
